Chilaiditi sign is a radiological finding of a large bowel interposition between the liver and right hemidiaphragm [ 1 ]. It can be symptomatic or asymptomatic; when the patient is having complaint, it is called Chilaiditi syndrome [ 1 ]. Caution must be taken, as this can be easily mistaken with pneumoperitoneum that considered one of the Abstract. Chilaiditi syndrome is a rare medical condition characterized by the interposition of a hollow organ, usually the colon, between the liver and diaphragm, leading to abdominal pain, discomfort, bloating, constipation, or nausea; in more severe instances, respiratory symptoms may manifest due to pressure on the diaphragm. Chilaiditi syndrome was first described in 1910 by Chilaiditi, a Greek radiologist ( 1 ). The incidence of this syndrome in the general population ranges from 0.025% to 0.28% ( 2 ). Unless the presence of volvulus is suspected, patients with Chilaiditi syndrome often respond to conservative treatments, such as nasogastric decompression.
